This new report comes at a pivotal moment for health and care. As the Government and NHS seek to shift care from hospitals into communities and place greater emphasis on prevention and neighbourhood health, Unlocking better care from hospital to home demonstrates the vital role that voluntary, community, faith and social enterprise (VCFSE) organisations can play in making this ambition a reality.
Drawing on evidence and learning from across the North West, the report sets out a practical framework for embedding VCFSE organisations within hospital discharge pathways and neighbourhood health models. It highlights how trusted community organisations can provide the practical, emotional and social support that people need before, during and after illness.
Alison Cullen, C&M VCFSE Health & Care Transformation Programme, said: “Every day, VCFSE organisations help people leave hospital safely, recover well at home and access support earlier, helping to prevent avoidable hospital admissions.
“This report shows that by embedding these trusted community organisations within health and care systems, we can improve outcomes for people while making better use of resources.
“The evidence is clear, investing in prevention and community based support is essential to building a more sustainable health and care system.”
The report demonstrates the significant benefits of investing in VCFSE led ‘step down’ support, with modelling in Cheshire and Merseyside showing that an annual investment of £2.88 million could unlock an estimated £13.38 million in savings for the health system, delivering more than £4 in value for every £1 invested.
